Out of state deposition procedures are relatively simple 

The request must be filed in the state level court in the jurisdiction where the witness resides or is employed. Whether the requesting venue has passed a Uniform Interstate Deposition and Discovery Act will dictate the required documentation for filing. If you are unaware of the requesting venue's stance, you may find it here

The required documentation to accompany the request depends upon the requesting venue and it's stance on The Act.
